When business owners start looking into alternative plan structures, the question that shows up (often from a well-meaning board member or anxious HR director) is the same one every time: isn’t self-funded insurance worse for employees?

The word “self-funded” suggests the employer is doing something unusual, something risky, and the employee is presumably on the hook for that risk. The reality is almost the opposite. Most self-funded plans are indistinguishable from fully-insured plans from an employee’s point of view, and in a lot of cases, they’re actually better.

The short answer

For the vast majority of employees at well-run companies with well-designed self-funded plans, the difference is invisible. They use the same doctors, show the same ID card, pay the same copays, and get the same federal protections. The plan might even be richer than the fully-insured version they had before, because the employer saved money and reinvested some of it in benefits.

The long answer has a few more nuances, which are worth walking through.

What actually stays the same for employees

The network. Self-funded employers almost always lease a provider network from a major carrier or network operator. Aetna, Cigna, BlueCross, and UnitedHealthcare all sell network access to self-funded plans. From the employee’s perspective, their doctor is in-network or out-of-network for the exact same reasons they were before. The underlying network is frequently the same one.

The ID card and claims process. The ID card might have a different logo or a different group number, but the process is identical. Show the card, pay the copay, the claim gets adjudicated through the same pipeline.

The essential health benefits. Federal law (the Affordable Care Act, which applies to self-funded plans through ERISA) requires coverage of preventive care, mental health and substance use treatment, maternity care, pediatric care, prescription drugs, and more. Self-funded plans cannot charge for in-network preventive services. They cannot impose annual or lifetime dollar caps on essential benefits. They cannot deny coverage based on pre-existing conditions.

The appeals rights. If a claim is denied, employees in a self-funded plan have the right to an internal appeal, an external review, and if the issue escalates, a federal lawsuit under ERISA. The legal protection is arguably stronger than what state-regulated fully-insured plans offer, because ERISA preempts state law and is consistently enforced.

The coverage for dependents. Coverage for kids up to age 26, same-sex spouse coverage, domestic partner rules: all governed by the same federal requirements.

What actually changes for employees

The employer, not the carrier, pays the claims. This is the structural difference. When a claim is submitted, the third-party administrator processes it, and funds are drawn from an account the employer funded. Most of the time, this is invisible. But it means the speed and reliability of claims payments ultimately depend on the employer’s cash flow and plan design.

In practice, this is rarely a problem. Well-designed plans use a claims account funded monthly and backed by stop-loss, and claims are paid on the same timelines as fully-insured plans. The situations where employees run into trouble are edge cases — an employer in financial distress, or a plan that was underfunded and hit a catastrophic year without adequate stop-loss.

Plan design can change at renewal based on actual experience. Under a fully-insured plan, if your group had a bad claims year, the carrier absorbs most of it and spreads the impact across their whole book of business. Your renewal will likely rise, but the benefits stay intact. Under a self-funded plan, a bad year is visible to the employer. They might respond by raising deductibles, increasing copays, or changing the plan design to reduce future exposure.

That’s the whole point of self-funding. The employer has line-of-sight into costs and can make informed decisions. But it does mean that plan benefits are tied more directly to the group’s own claims experience.

Potential for richer benefits. On the upside, self-funded employers frequently use some of their savings to improve benefits. Lower deductibles, better prescription coverage, expanded mental health benefits, first-dollar coverage for primary care. All of these are easier to offer when the employer is keeping the savings rather than handing them to a carrier.

The risks employees should understand (and the protections that exist)

There are three real, if rare, risks to employees in a self-funded plan.

1. Employer insolvency. If the employer goes out of business mid-plan-year, what happens to outstanding claims? Under ERISA, unpaid claims become an employer debt in bankruptcy. Most employees are made whole through stop-loss run-off coverage or a new carrier taking over at acquisition. But in rare cases, there can be delays.

The protection: well-designed plans maintain adequate claims reserves, use run-out stop-loss coverage, and don’t skimp on the financial stability of the administering carrier.

2. Underfunded stop-loss. If an employer chooses a very high specific stop-loss attachment point to lower their monthly cost, a single catastrophic claim could strain the plan’s cash position. In theory, this could delay claims payments to the employee.

3. Plan-design cuts at renewal. If claims come in hot, the employer may reduce benefits or raise employee contributions next year. This isn’t unique to self-funding (fully-insured employers also respond to bad renewals by cutting benefits), but the connection between experience and renewal is more direct under self-funding.

The real question isn’t “self-funded or fully-insured.” It’s “is this plan well-designed and well-managed?” A thoughtful self-funded plan is better for employees than a sloppy fully-insured one, and vice versa.

Why self-funded plans are often better for employees

The case that self-funded plans are often better for employees boils down to three things:

Premium stability. Fully-insured premiums tend to rise year over year, well above general inflation, regardless of an individual group’s actual experience.1 Self-funded plans tied to your own favorable experience produce more stable year-over-year costs, and the savings can be passed through to employees in the form of flat or reduced payroll contributions.

Better benefits for the same money. Savings from moving to self-funding can fund a richer plan design. Lower deductibles. Lower out-of-pocket maximums. Expanded telehealth. Better mental health access. The employer decides how to deploy the savings, and many choose to reinvest in the employee experience.

More transparency and advocacy. Self-funded employers, especially smaller ones working with a modern advisor, are more engaged with how care is actually delivered. What employees should ask HR

If your company announces a switch to a self-funded or level-funded plan, the questions to ask are practical:

  • Is the network the same? (Usually yes — check the carrier logo on the new ID card.)
  • Are my doctors still in-network? (Usually yes — confirm in the new network directory.)
  • Are the covered benefits the same or different? (Check the Summary Plan Description.)
  • What happens if a claim is denied? (ERISA appeals process — HR can walk you through it.)
  • Who processes my claim? (The third-party administrator — often the same national carrier as before.)

Employees who ask these questions almost always get reassuring answers. The structural change is quiet, and in a well-run plan, it should be.

What employees actually experience

Is self-funded health insurance good for employees? For the overwhelming majority of people at well-run companies, the answer is yes. In many cases, self-funded plans deliver a better experience than the fully-insured plan they replaced.

The risks exist but are rare and manageable with competent plan design. The benefits (premium stability, richer plan design, better care navigation) are real and compounding.

Where employees get hurt is when plans are poorly designed: inadequate stop-loss, underfunded reserves, or employers who see self-funding as a way to squeeze benefits rather than invest in their team. The structural choice isn’t what determines employee outcomes. The people running the plan are.
