Walk into most broker meetings and you’ll hear about two options: the fully-insured plan you already have, and maybe — if you’re a big enough company — a fully self-funded plan. What you rarely hear about is the option sitting in between: the level-funded health plan.

Level-funded plans generate lower commissions for brokers, and they require a bit more explanation than the off-the-shelf fully-insured product. But for thousands of small and mid-sized employers, they’re the structure that actually fits: all the upside of self-funding with almost none of the cash-flow variability.

What is a level-funded health plan?

A level-funded health plan is a self-funded health insurance arrangement packaged into a fixed monthly payment. Every month, the employer pays the same predictable amount (the way you would under a fully-insured plan), but under the hood, the money is split three ways:

  1. Expected claims fund. A portion goes into an account dedicated to paying your team’s actual medical claims.
  2. Administrative fees. A fixed percentage covers the third-party administrator or carrier that operates the plan.
  3. Stop-loss premium. A portion pays for the stop-loss insurance that caps your exposure to catastrophic claims.

At the end of the plan year, the claims fund is reconciled. If your team’s claims came in below the expected amount, many carriers refund the surplus back to the employer (rules vary by carrier and state, but refund structures are common). If claims came in higher than expected, stop-loss covers the excess. You’re not on the hook beyond your monthly payments.

The whole point of the structure is to give you the predictability of fully-insured cash flow and the transparency and upside of self-funding, at the same time.

Level-funded vs. fully-insured: what actually changes

The day-to-day experience is almost identical. Your employees use the same provider networks, show the same ID card, and go through the same claims process. The difference lives entirely on your side.

What stays the same:

  • Fixed, predictable monthly payment
  • Provider network access
  • Employee experience
  • Compliance with ACA requirements

What changes:

  • You see your claims data (which is yours legally, but fully-insured carriers rarely share it)
  • You can adjust plan design with more flexibility at renewal
  • You keep any savings from low-claims years
  • You don’t pay state premium tax on the claims portion (varies by state)

For healthy groups, level-funded quotes come in below comparable fully-insured quotes in the first year, with any surplus refund providing additional upside if claims come in low.

Level-funded vs. self-funded: what actually changes

If level-funded is technically a form of self-funding, what’s the real difference?

Cash flow. A fully self-funded plan pays claims directly as they come in. That means your monthly spend varies: some months are quiet, some are expensive. Level-funded smooths that into a predictable bill.

Administration. Level-funded is essentially a packaged product. The carrier handles the bundling, the stop-loss negotiation, and the surplus calculation. Fully self-funded typically involves a third-party administrator and more active management from the employer or broker.

Customization. Fully self-funded plans can be designed with more granular control over benefits, networks, and claim-handling rules. Level-funded plans are more standardized. You pick from the carrier’s menu of options.

Stop-loss control. In a fully self-funded plan, you shop stop-loss separately and can negotiate attachment points and terms. In level-funded, stop-loss is bundled with the plan.

For employers with 10-50 employees, level-funded is almost always the better starting point. For employers with 100+ employees and strong claims experience, a fully self-funded arrangement may deliver more savings. The size cutoff isn’t rigid. It’s a function of your claims profile, risk tolerance, and finance team’s appetite for variability.

The question to ask isn’t “level-funded or self-funded.” It’s “am I still on fully-insured, and why?” Because both alternatives give you what fully-insured structurally can’t: visibility.

Level-funded health plan pros and cons

Pros:

  • Fixed monthly cost. Same budgeting predictability as fully-insured.
  • Claims data access. You can finally see what’s driving your spend.
  • Potential surplus refund. Good years put money back in your pocket.
  • Often cheaper than a fully-insured quote for a healthy group.
  • ACA-compliant by default; employees see no difference.
  • No state premium tax on the self-funded portion (state-dependent).
  • Accessible at smaller group sizes (10+ employees in most cases).

Cons:

  • Medical underwriting required. Most carriers require health questionnaires or claims history to quote. Groups with known high-cost conditions may see worse pricing or be declined.
  • 12-month commitment. Once you’re in for the plan year, you’re in. Switching mid-year is rare.
  • Renewal re-underwriting. If your actual claims experience was bad, your renewal will reflect it. Unlike fully-insured, where a bad year is spread across the whole pool, here it’s yours.
  • Refunds aren’t guaranteed. If claims exceed the expected amount, there’s no surplus. Stop-loss protects you from going upside-down, but the refund is a potential outcome, not a promise.
  • More engaged plan management. You should be reviewing claims reports, not just signing a premium invoice each year.

When a level-funded plan is the right fit

Level-funded is the clear winner for groups of 10-200 employees with healthy or favorable demographics, where the finance team wants fixed monthly payments and leadership wants actual visibility into costs. It’s an especially strong move for employers sitting on fully-insured plans facing steep renewal increases, or those planning to move to fully self-funded in a year or two once they have their own data.

It’s the wrong fit for groups under 10 employees (the stop-loss math gets thin), groups with known chronic high-cost conditions, or companies where leadership has no interest in engaging with claims data. If you can’t tolerate the risk of a rough renewal after a bad claims year, fully-insured may be the more comfortable structure.

For businesses in the first camp, the opportunity cost of staying on fully-insured is almost always larger than the risk of trying a level-funded plan. One bad year under level-funded is rare and bounded by stop-loss. Ten straight years of carrier markup under fully-insured is the norm.

How level-funded plans handle stop-loss

Stop-loss is the piece that makes this work at the small-group level. Without it, a single catastrophic claim could blow up the plan budget. With it, your worst-case outcome is defined.

A level-funded plan bundles stop-loss into your monthly payment. The carrier sets two limits:

  • Specific stop-loss kicks in when any one individual’s claims exceed a defined threshold for the year
  • Aggregate stop-loss kicks in when the whole group’s claims exceed a defined ceiling, expressed as a percentage above projected claims

How to evaluate a level-funded quote

When you receive a level-funded quote, don’t just compare the monthly premium to your current fully-insured number. Look at the full structure:

  1. What’s the stop-loss attachment point, both specific and aggregate? Lower thresholds mean more protection and a higher monthly cost.
  2. How is surplus calculated and returned? Is it a percentage split with the carrier? Is there a holdback? Is it distributed as a lump sum or applied against next year’s premium?
  3. Is there lasering? Lasering is when the carrier carves out a specific high-risk individual with a higher personal attachment point. It can dramatically affect your exposure.
  4. What’s the renewal process? Ask the carrier directly: how do you re-underwrite at renewal, and what’s a typical renewal increase for a neutral-claims year?
  5. Are you pre-paying a year of claims? Some structures require the employer to fund the full expected claims amount upfront. Others prorate monthly. Cash flow matters.

Where this leaves you

A level-funded health plan is the structure most small-to-mid-sized employers should be comparing against their fully-insured quote every single year. It gives you the predictability your CFO wants, the data your plan advisor needs to actually help you, and the upside of keeping what you don’t spend. The trade-off is bounded claims risk (capped by stop-loss) and more active engagement with the plan.
